Work and Fibro, Treatment not working

I am 20 years old and was diagnosed with Fibromyalgia 3 months ago. I was already taking Nortriptylline so I was advised to up the doseage of that to treat the fibromyalgia. Even though I have been on the medication for a long time now, I still cannot sleep, the pain in my body is so bad sometimes I can barely walk, I have migraines and lose my vision, I suffer from bad depression and I am now very forgetful and cannot get my words out somedays.
As a result of this, I am missing an awful lot of work, to a point where I feel work are becoming very angry with me, I have considered handing in my notice, as I feel I cannot cope with work anymore, on the days I do go to work, i come home in tears, I am exhausted and in alot of pain. Has anyone else had this problem where they feel they are unable to work? if so, how did you approach your employer? I am looking for any advice to help me through this time, as I am at a loss and have no idea where to go from here.

Hi Holly. I do not know about your job and its benefits but look up your companies benefits. You can use your vacation time and sick leave. You can also use your companies disability leave but it is alot of paper work and it has to go through your doctors. Are you diagnosed and have paperwork on your illness? If so approach the benfits department and human resources and explain the problem after talking with your doctor and get the ball rolling that way.
