Is there any way that a doctor can give a written report to prove that you have fibromyalgia my long term disablity do not want to contuine to pay for this condition

OF COURSE! they would have had to write something in the first place or you would have never gotten any money just 'saying' you had FM!!! ask the doctor for god's sake! they will probably charge you (of course) but who cares. how did you get long term disability in the first place my dear?

Yes, if you have FMS (or any condition) you will need a report from your doc in order to qualify for disability. I am not sure what success you will have in qualifying. Depends on how severely you are disabled from it. Would like to hear if you are successful, though.

The doctor did fill out the forms but now the say they wont pay because fm is a self reported systoms and it cant be proven by any doctor

I live in Canada, I have no problem with approval here. this seems to have a lot to do with the US's shitty medical system. Fibromyalgia is a diagnosable illness! there is a tenderness point test & other criteria. you can fight this! unfortunately it really seems to be a matter of geography as to whether you get the help you need. such bullshit! ask for a referral to a rhumetologist or a physiatrist, they are specialist who diagnose fibro. they can't dispute it coming from them, a GP maybe, but not a specialist! good luck! let us know how it goes :)
